Position Overview: We are seeking skilled and compassionate Occupational Therapists to join our team. In this role, you will work closely with older adults, persons with disabilities, and their families to assess their homes and recommend modifications, DME, AT, medication routines, and balance and strength training programs to enhance safety and independence.
Key Responsibilities:
Conduct Comprehensive Assessments: Perform thorough evaluations of clients and their homes to assess their functional limitations, mobility challenges, and safety concerns.
Customized Modification Plans: Collaborate with clients, their families and construction specialists to recommend DME, AT and develop personalized home modification plans based on assessment findings.
Recommendations and Solutions: Provide detailed recommendations for home safety improvements, home modifications, DME, and AT to address clients' unique needs.
Coordination: Work closely with care managers, service coordinators and construction experts to ensure the successful implementation of home safety assessment, AT and home modifications insurance benefits.
Documentation and Review: Utilize the efficient Vest EHR (no OASIS!) to maintain accurate and up-to-date records of assessments, recommendations, and progress reports in accordance with program requirements.
Follow Up Visits and Client Education: Perform in-person follow-up visits after the modifications are completed and the DME or AT equipment is delivered. Educate clients and their caregivers on the proper use of recommended equipment and modifications to maximize their independence and safety.
Stay Current: Stay updated on the latest advancements in home safety, DME, modification technology, accessibility standards, and healthcare trends.
Requirements
Qualifications:
Master's degree or Doctorate in Occupational Therapy from an accredited program.
Valid Pennsylvania state licensure and NCBOT certification as an Occupational Therapist.
Available for in-home safety assessments and home modification evaluations throughout Southwestern Pennsylvania. We are especially looking for OTs to serve clients in the following areas: Eastern Allegheny, Westmoreland, Crawford, Venango, Mercer, Butler, and Fayette counties.
Certification or specialization in home modification and accessibility is a plus, (CAPS, ATP, ECHM, CLIPP, AOTA Home Modifications Online Badge).
Strong interpersonal and communication skills.
Detail-oriented and organized.
Ability to work independently and as part of a multidisciplinary team.
Valid driver's license, reliable transportation, and proof of auto insurance.
